Delivering NDIS Supports Your Way

Enabling people with disability and their family to shape the service they receive is a natural and necessary component of person-centred practice.

We recognise that facilitating responses to meet the needs, wants and aspirations of people is not just about the relationships between support workers and the people they support; it is a culture of mutual respect that honours and recognises the unique interests, preferences, talents, and life experiences of each person involved in planning and delivering support.

We have developed numerous tools and procedures to ensure control is in the hands of people with disability and their families. The most notable is our person centred planning process – The 4D model.
The 4D model of; Discover, Design, Deliver and Develop is an immersive and iterative approach to providing supports to people with disability.

Discover Phase

Initial Contact & Referral

After receiving your enquiry one of our friendly team members will reach out to you directly over the phone to learn more about you.

We’ll then need to make things a little more official and we’ll ask you to fill in a referral form – it’s a big form (YAWN!) So, if it’s your preference, we’ll work with you over the phone and do it with you. This extra information will help us to define if we are the best fit for you and you for us.
Think of it as a bit of a first date 🙂 .

Discover Phase

Review & Request

After we have your referral form we get to work, to ensure that we are the best fit and can support you the way you want. This sometimes means we need extra information or a Functional Capacity Assessment (FCA). We’ll also ask you for consent to hold information about you and confirm funding.

Decision & Meeting

Once we confirm a few details and assess your needs and our capacity we’d like to meet you and give you a chance to meet us. We also encourage you to contact our other customers and ask questions so you feel assured.

If you feel like we are the right provider for you, you will receive a service agreement for signing then a welcome pack and other resources.

PATH Planning & Goals

Now we’re cooking! Our person-led approach becomes apparent at this point. We schedule some time with you and the people closest to you. We listen and learn from YOU! What you want and what’s your north star? We use PATH planning techniques to ascertain this and work alongside you each and every step.

Your System

While the above is happening we start building out a system tailored to you. Only CSBS, your support team, and the people you elect can access it.

We use this system to track data, keep progress notes, see how we’re tracking against your goals, and as a central information hub so that your support team can stay up-to-date. Best of all, it’s useful tool that provides evidence at your next funding round.

Deliver Phase

Business As Usual & Customisation

At this stage, we deliver as we’ve co-designed with you. You’ll be supported YOUR way by the supports YOU have chosen.

However, we also like to use this time to improve, get better, and work alongside you to be… well… just better. We’ll maintain progress notes, stay in communication, and work closely with your support team to ensure you feel empowered and are working towards your goals.

Develop Phase

Always Improving

As we learn from each other and deliver supports your way, we’ll seek to improve. We’ll schedule check-ins to see how/if we need to adapt to any changes and, to see how you are tracking with your goals. Based on these catch-ups we’ll ask if ‘If you feel like you’re being supported the way you want?’ then we’ll make adjustments to ensure you are. Then it’s a case of iterating and improving over time to deliver the best supports customised to you.
