Jane's Story.

From hospital to home

A Safe Place to Begin Again

CSBS collaborated with hospital teams, coordinated safe housing, and delivered training so her support team was prepared from day one.

By leading with compassion and capability, we worked with Jane to rebuild trust, feel safe again, and slowly regain her independence.

Today, Jane is walking, smiling, and no longer just surviving – she’s living.

Sienna's Story.

Data-informed advocacy

Making the Invisible Visible

CSBS introduced structured data collection to track her seizures, care needs, and risks.

This evidence told a story no paperwork had before.

The result? A compelling case that secured increased funding and clinical support.

Sienna now lives more safely, with dignity and support that reflects her reality.

Complex Needs, No Plan

Sienna came from a background where her needs hadn’t been met for a long time. She was medically complex but didn’t have any formal supports or funding.

Immediate Risk & Clinical Needs

Her team started tracking key things — like her seizure patterns — so they could better understand what was happening day-to-day and share that with the people who needed to know.

Making the Case for More Support

CSBS implemented a data-led approach. Our team began collecting detailed records on seizure frequency, duration, and intensity. This was visualised through graphs and tables to build a clear picture of daily challenges.

Data driven advocacy

With this evidence in hand, we were able to advocate for increased funding to match the real-world complexity of Sienna’s care needs. The data created the visibility for what was previously hidden. Sienna now receives appropriate nursing support and funding that reflects the reality of her needs.

Tilly's Story.

Person-led... always

Connected with place and culture

Tilly felt adrift. Isolated from her culture and community. Her behaviours were escalating, and her support wasn’t aligned to who she really was.

Our team stepped back and listened… really listened.

We reshaped her supports around her identity, culture, and values.

The transformation was powerful: she reconnected with her community, found stability, and isconnected to her culture one more.

Feeling Disconnected

Tilly had been living in a place that didn’t feel right for her. She was showing more behaviours and had lost touch with her cultural roots and community.

Support Needs & Behaviours

Her team took a step back to listen and learn more about who Tilly is — her background, her culture, and what really matters to her.

A Person-Led Support Reset

With a better understanding, the support around her was reshaped — to be more respectful, more consistent, and more connected to her values and goals.

Cultural Reconnection

Tilly’s now in touch with her community and getting ready to travel back to Country to visit family. It’s a big step for her, and one she’s excited about.

Her behaviours have settled, she’s more confident, and her supports reflect who she is — not just what services were available.

James' Story.

The Golden Thread in Action

Holding the Line With Evidence

James required high-intensity behaviour support and could not live safely with others. When policy changes downgraded his plan to standard support, his hours were gradually reduced across three reviews — putting his independence and wellbeing at risk.

In response, we implemented a customised data-tracking system that captured daily incidents, staff interventions, and specific care needs — revealing the full extent of support required.

This targeted evidence was submitted to the NDIA, clearly justifying the need for high-intensity funding, specialised staff training, and one-on-one supervision. 

As a result, James’s high-intensity funding was reinstated, and he continues to live safely and independently — on his terms.

Downgraded Support, Rising Risk

James had previously received funding for high-intensity behaviour support and lived independently. But changes to NDIS criteria resulted in his funding being downgraded to standard — and over three plan reviews, his support hours were progressively cut.

Hidden Complexity, Custom Data

While James’s needs hadn’t changed, the system no longer recognised them. We built a tailored data-tracking process to capture what generic reports missed — daily incidents, staff interventions, and high-risk moments.

Building the Case

The daily data clearly showed that James required specialised support, additional staff training, and one-on-one supervision. It also evidenced that shared living would place him — and others — at risk.

Reinstated and Respected

The NDIA accepted the evidence. James’s high-intensity funding was reinstated, along with the hours and structure needed to keep him supported as needed.

Independence, Protected

Thanks to targeted data advocacy, James continues to live where he chooses — safely, with dignity, and with the support his reality requires.

Carl's Story.

Support Without Distance

Staying Connected, Across Continents

When Carl’s partner, Vivian, spent part of each year overseas, we ensured distance didn’t mean disconnection.

Through secure access to CSBS Connect and Microsoft Teams, Vivian remains involved in Carl’s day-to-day life. With real-time updates, photos, and communication that keep her close, wherever she is.

A Life Shared Across Borders

Carl lives in Supported Independent Living. His partner, Vivian, lives with him part-time in Australia and part-time overseas. But physical distance didn’t change her desire to stay involved in his care.

Creating a Bridge, Not a Barrier

To support this connection, we set Vivian up with secure access to CSBS Connect, allowing her to view Carl’s real-time progress notes and behaviour records as they’re submitted by the support team.

Transparent, Human Updates

Using Microsoft Teams, Vivian receives messages and regular photos from staff — showing Carl’s meals, community outings, and daily routines — keeping her emotionally connected and informed.

Connection as Care

This approach allows Vivian to remain an active, valued part of Carl’s support circle — even while overseas — without needing to chase updates or feel disconnected.

A Model for Inclusive Support

Carl continues to thrive in his home, and Vivian remains a consistent part of his life. Together, they benefit from a support model that recognises relationships as essential, not optional.
