The Future of Disability Support is Evidence-Based
More than support. Meaningful evidence.
As the NDIS evolves, the ability to demonstrate support needs, outcomes and changes over time is becoming increasingly important.
Most providers record support activities.
CSBS has developed a structured support measurement and evidence system designed to turn everyday support interactions into meaningful information that can assist participants, families, Support Coordinators and allied health professionals to better understand support needs, outcomes and change over time.
Better information. Better decisions. Better outcomes.
The NDIS IS Changing...
- Functional capacity
- Participant-specific evidence
- Support outcomes
- Changes over time
- Record keeping and accountability
- Demonstrating why supports are required
Can your provider clearly demonstrate the support you need?
A provider may deliver excellent supports every day.
But if support needs, changes, risks and outcomes are not properly documented, opportunities to demonstrate the true impact of disability can be missed.
The Difference Between Notes and Evidence
Notes tell a story. Evidence shows a pattern.
A single support note only tells us what happened on one day. Hundreds or thousands of support interactions collected over time can reveal:
- support needs
- changes in functional capacity
- emerging risks
- participation patterns
- increasing support requirements
- the outcomes of support delivery
That longitudinal picture is often far more valuable than isolated observations.

I-CAN v6 Is Coming. We’re Preparing Now.
One assessment captures a conversation. Good evidence helps capture the whole story.

What’s changing

The genuine concern
One assessment conversation cannot automatically replace the knowledge of the person, their family, trusted allied health professionals and the team supporting them every day.

The CSBS approach

The important distinction
CSBS does not conduct the official NDIS assessment or determine funding. We help ensure the person and their wider team are as informed and prepared as possible.
Harold's Story
What Happens When Everyday Support Becomes Meaningful Evidence?
Meet Harold.
Harold lives in Supported Independent Living.
At first glance he appears mostly independent.
However, over months of support delivery, records begin to show:
- increasing prompting requirements
- longer completion times
- disrupted sleep
- greater reliance on support workers
- growing overnight support needs
No individual record tells the story.
The pattern does.
NB: Harold is a fictional example created to demonstrate how structured support information can help build a more complete picture of support needs over time – Harold’s image was made using AI.
New Provider Selection Checklist
Can they demonstrate what support was provided?
Can they show outcomes over time?
Can they identify changing support needs?
Can they support reviews and planning discussions?
Do they understand complex supports?
Can they produce meaningful evidence, not just progress notes?
