James required high-intensity behaviour support and could not live safely with others. When policy changes downgraded his plan, his hours were gradually reduced across three reviews – putting his independence and wellbeing at risk.
In response, we implemented a customised data-tracking system that captured daily incidents, staff interventions, and specific care needs…
