NDIS Hospital to Home Support
Because Everyone Deserves More Than a Bed
Moving from hospital into a Supported Independent Living home is about more than finding a place to live. It’s the beginning of a new chapter. The right home provides stability, security, connection and the opportunity to build a life that feels meaningful and fulfilling.
At CSBS, we work alongside people, families, nominees, healthcare professionals and support teams to create smooth transitions from hospital into homes where people can feel safe, supported and truly at home.

Why Hospital to Home Matters
Hospital transitions can be overwhelming.
Questions often come quickly:
- Where will I live?
- What supports will I need?
- Is the home the right fit for me?
- How will everyone work together?
- What happens after I discharge?
Our role is to help answer those questions and guide a pathway forward.
We bring together housing, support, clinical considerations and everyday life so people can move forward with confidence in a new home.
What We Do Best:
- Support the move from hospital into a home where you can thrive
- We Have Supported Independent Living (SIL) vacancies available now
- Experts in Complex care and high-intensity support
- Work with and alongside families, nominees and support networks
- Collaborative planning with hospital and healthcare teams
- Tailored transition planning and coordination
- Build confidence, connection and independence
- Create stability through the right home and supports
- Strictly Person-led support designed around individual goals
- Ongoing support beyond discharge day and the reporting to provide ongoing evidence.
Our Approach To Hospital To Home Supports
1. Understanding the Person
Every situation is different. Before discussing housing or supports, we take the time to understand the person, their goals, what matters to them and the life they want to build.
2. Exploring the Right Options
3. Planning the Transition
We help coordinate the practical details that support a safe and sustainable transition.
4. Ongoing Support At Home
Life doesn’t stop at discharge. Our teams continue working alongside people long after they arrive, helping build stability, confidence, connections and independence.
Supporting NDIS Complexity
Our teams have experience supporting people living with:
- Acquired Brain Injury
- Neurological conditions
- Physical disability
- Complex health needs
- Behaviour support requirements
- High-intensity support needs
- Psychosocial disability
We understand that complexity looks different for everyone.
That’s why every support arrangement is tailored to the individual.

A Safe Place to Begin Again
When Jane* was admitted to hospital, she was facing more than a health crisis.
She had no supports in place, no family nearby and no clear pathway forward.
Working alongside hospital teams and other stakeholders, CSBS helped coordinate safe housing, prepare supports and create a transition plan that focused on stability, safety and a fresh start.
Over time, trust was rebuilt. Jane developed strong relationships with her support team, regained confidence and began rebuilding her independence.
Today, Jane is no longer simply getting through each day.
Jane’s living life on her own terms.

“A successful transition isn’t measured by discharge day. It’s measured by what happens afterwards.”
