Hospital to home transition for NDIS Participants.
Our Hospital to Home Transition Coodinator will support your transition from hospital to a home with the right supports for your needs.People with disability often find themselves staying in hospital longer than necessary simply because they cannot find suitable accommodation that meets their needs so they can leave hospital.
Our hospital to home coordinator works with people with disability who find themselves in this type of situation. We will meet with you, your family or carers, your medical and discharge team, and your support coordinator to understand your specific health needs when leaving hospital and the types of support that will best meet those needs.
We will identify the full range of supports you will need when you leave hospital. For example, you may need nursing care at home while you are recovering from an illness or surgery, or a house with wider doorways to accommodate a wheelchair or walking frame, lower kitchen benchtops, or accessible bathrooms, or support workers to help you with showering and dressing or cooking meals. We will follow your discharge plan and make sure we understand your needs so you are comfortable, supported, and happy at home.
If you or someone you care for is a NDIS participant and need to find housing and support so they can be discharged, we will find housing and a team of support workers to provide the care they need.
We are a registered NDIS provider specialising in supporting people with complex needs whether it be for designated hours on certain days or around-the-clock care.
We provide services and supports delivered by trained and experienced staff to support people with disability needing:
- Supported Independent Living (SIL)
- Specialised Disability Accommodation (SDA) – accommodation with design features based on a person’s disability support needs
- High physical support – where a ceiling hoist, backup power supply, home automation or communication technology etc are needed
- Community Nursing Care –post-operative care and wound management and other medical and health support services such as diabetes management, complex bowel care etc
- Activities of daily living – home help, showering and dressing, cooking, and cleaning
- Community participation – going to appointments, shopping, and community activities
- Positive Behaviour Support
